Where Will Married Women Receive the SIR Form?

The Booth Level Officer (BLO) will deliver the form to your registered address as per the 2025 voter list.

  • If your name is still listed at your parental home, the form will reach your parental home.
  • If your name has already been transferred to your in-laws’ home, the form will be sent there.

So, the form will be delivered based on your current voter registration location.

What Details Will You See on the SIR Form?

At the top of the form, you will notice the title “Election Commission of India – Summary Revision Form”. The form will include:

  • The name and contact number of your BLO
  • Your full name, voter ID number, village, and constituency details
  • A QR code for verification
  • Your current voter card photograph
  • A blank box for your new coloured passport-size photograph

Paste a clear and recent colour photo in the given space. This new photograph will appear on your updated 2026 voter card that will be sent to your home.

How to Fill Out the Date of Birth

  • Write your date of birth (day, month, and year) exactly as it appears on your 2025 voter card.
  • If your date of birth is incorrect, write it as shown on the card. This form is not for making corrections.
  • Always write numbers in English, not in Bengali.

Aadhaar and Mobile Number

  • Write your Aadhaar number (if available) in English digits.

Understanding the Left and Right Boxes in the SIR Form

In the SIR form, two main sections exist, the left box and the right box. You need to fill out only one of them.

  • If your own name was in the 2002 voter list, fill out the left box.
  • If your name was not in the 2002 list, fill out the right box, using information from your father, mother, grandfather, or grandmother’s 2002 voter list details.

You can only mention the name of one of your four closest relatives from your parental side:

  1. Father

  3. Grandfather
  4. Grandmother

You cannot use your husband’s 2002 voter list.

  • Find the 2002 voter list from your parental home’s polling booth.
  • Write your relative’s name exactly as it appears in that list.
  • Enter their EPIC number, if available.

In the “Relationship” box, write the correct relation:

Then, fill in the district, state, constituency, part number, and serial number based on the 2002 list.

Signature or Thumb Impression

At the end of the form:

  • Sign your name clearly.
  • If you cannot sign, give your thumb impression.
  • If you are not at home, an adult family member (18 years or older) can fill and sign the form for you.

They must write their relationship to you and the date. After submission, the BLO will verify and sign the form.

  • One copy will be taken by the BLO.
  • The other copy will remain with you as a receipt.
